Steps Involved in Evaluations

Before the Appointment

Prior to the first evaluation appointment, please complete the paperwork in the Forms section. You can bring the forms with you to our initial appointment; however, if your insurance requires pre-authorization, please send the completed Fee Agreement and Developmental History forms to me at least 3 weeks before we meet.


Evaluation Appointments

An evaluation begins with a meeting with parents, which lasts approximately one hour. During this time, I will gather background information, answer questions you may have and discuss the plan for the evaluation to ensure I cover all the areas you would like assessed. A comprehensive evaluation typically involves the child participating in 5 to 6 hours of face-to-face testing time. The testing sessions typically occur over one to two meetings, depending on how much testing is needed and the age and stamina of the child. We take regular breaks during the testing including a lunch break (please pack lunch and snacks).


Gathering Perspectives

In addition to gathering the impressions of the child and parents, it is helpful for me to gather the impressions of others who know the child well, such as teachers, therapists and doctors. Please complete release of information forms for anyone you would like involved in the evaluation (located in the Forms section).


Reviewing Assessment Results

Once all the testing is completed and I have interpreted the results and put all the information together, we will have a feedback meeting to review the results and recommendations. I typically present the results using a visual aid as I have found this helps with understanding the information. I also provide the results and recommendations in a written report. The feedback meeting typically occurs approximately 3 to 4 weeks after our testing sessions and lasts about 1 hour. Depending on the age of your child, we may have your child involved in this feedback meeting or we may schedule a separate meeting to explain the assessment results to your child in a way that is age appropriate.
